If you have one with LTE there is a section for it. I had the Pixel Watch 2 with LTE. I never used it and never needed it. It uses an eSIM but not all MVNO support it on the watch.
I upgraded to the Pixel Watch 3 with WiFi only and it's fine for me. You may need LTE. I think it's around $50 more

Judgment call. Actually, as another birthday hit today, I don't run at all. I usually have me phone but occasionally it is elsewhere like in my paniers when biking and I have answered the "phone call" on the watch. But infrequently actually. It is probably more of the novelty of it all. It reminds me of Dick Tracy and his wristwatch phone. And the money I can't take with me but I can take the watch at the moment.Is that Worth it to you? Do you run without your watch?
