Bionic SD Card Troubles

I have a 32GB SD card that shows up as blank SD and every time I go to format it, the phone freezes and reboots itself. I tried putting it into my computer via a USB reader and it will not show up. The phone notices its presence but the computer doesn't. I cannot get the phone to format it.

Any ideas for a solution??
