Boost Mobile HTC phone trouble applying OTA update for Android 4.2

I just found out there is an OTA update for my phone and when I first tried to install it, CWM was giving me an issue. I flashed the stock recovery.img and retried but now instead of the error CWM was giving me, my phone acts like it's about to install the update then gives me the red triangle.

My phone is a Boost HTC One SV, unlocked, rooted, s-off.

I am having a similar issue... I have a rooted HTC one SV boost s off with moonshine running TWRP recovery. It will not let me flash the OTA it gives me E:Error executing updater binary in zip'/int_sd... What do I need to do to update the OTA's? Any help would be appreciated. Thank you
