Camera lens

So my rear camera lens is really scratched up now. I really have no idea how. I've had case on this phone so the camera lens doesn't really make contact with anything. Also my front camera takes pictures as if there's a permanent fingerprint smudge on it no matter how much I wipe it. I have insurance with Sprint. Would I be able to walk into my Sprint store and be able to swap it out for another one? Have any of you had camera lens issues?
