Can't get MMS to work on H2O

I'm having trouble sending and receiving pictures on my new moto g 2nd Generation. Anyone doing this on H20 and would be willing to share their settings with me?
 
If you are using Consumer Cellular, this will happen if the APN settings are incorrect or if they don't have the phone's IMEI. Call them and they will walk you thru fixing it.

APN Type: default,supl,mms
 
What are APN the settings for a Nexus 5 on H20?
Do I type in "blank" for user name, password, and server or just leave "not set"?
