Anyone have any suggestions for a case that can handle the extended battery? Preferably one that also incorporates a screen protector.

Skinomi has screen protectors out for it already. I ordered one this morning.

Used them on my Pre Plus and Dinc, I like them. A lot of people complain that the "Orange peel" texture doesn't look or feel right.. I wore mine both smooth after 3-4 weeks.

Is that a single layer, and a skin protector a double layer?

The ones I have used in the past are all wet application single layer.

Heard back from Phantom Skinz. They're going to have one, but, at the moment, don't have the phone to draw it. They're looking to borrow the phone and offer a free skin in return.
