Dead phone and yellow info

Hi I'm having trouble with a Samsung galaxy note 2.... 1 my first problem is I can only use it when it's on chargewhen I unplug the charger my phone dies imeditly after unplugging.... it goes to 100% to dead and want come on in a slip second.... is it the battery?
2 my second thing is that when it does come on it shows a list of phone info in yellow letters right in the middle of the phone on home screen and everything else... what can I do to make the yellow info go away and keep my phone from being dead dead dead.... lol
