That usually means that the default location for apps to save data or to download files (when possible) will be the SD card, not Internal Storage.
Sorry, I don't quite understand the question. Changing which settings? I wouldn't see any need to factory reset your phone if you're just switching the default storage from Internal to SD.
No, you only need to reformat it if you want to format the card as Internal Storage (i.e., Adoptable Storage, where the card is actually considered to be part of Internal Storage by the system). Not all manufacturers support this. If you don't see an option to format an SD card as Internal Storage, then the device doesn't support that feature.
That's a good question, though. If an app already saved some data to Internal Storage before you changed default storage to the SD, I'm not sure if the app will then start saving data to the SD, and keep that previously stored data on Internal -- or if it will move everything to the SD.
