Disappearing icons

Every few days icons disappear from my home screens. Sometimes they wind up in folders and sometimes their just gone.. since I have no app drawer this becomes a pain in the butt
Any ideas
In folders no problem
Not in folders have to find and re -download
The funny thing is that it could be a day or so before I realized what's happened lol

Do you have any kids?
Sorry, just the first thing I have thought of, specifically kids that have access to your electronic device. I have seen it many times.

No
Only thing on SD card is LG backup

Only time I have that issue is during restarts. And its only the second screen that will do it.
