Driving mode and bluetooth problems

I have my phone setup to go into driving mode as soon as it detects the bluetooth in my vehicle. But when I get a text or call, it won't read out the message. I can make it work out of the car but as soon as I get connected in the car it stops working. Also if I can get it to work, is it supposed to read over the phone speakers or can I make it work over the speakers in the vehicle? I have done some searching on this but still can come up with a answer.
