Do you have to set up email etc every time?

Depends. If you are running a rom, you can usually upgrade that rom without wiping. If you are switching different ROMs, then you will have to wipe.. If you backup via titanium, you can save the app and data of most apps onto your sd card and restore them. Takes a lot less time than downloading them from the market and your angry birds progress is saved

If you save data in titanium in rom x then switch to rom y is it still safe to backup data in titanium?

Most of the time, yes, it will work. Sometimes it doesn't... just make sure you do a nandroid backup (complete backup in the recovery) and you could always go back to rom x just as it was before. I personally like my root backup better than titanium.
