Galaxy S5 - I intermittently get the 'eye' icon appearing in my notifications. How can I rid myself of this icon

Galaxy S5 - I intermittently get the 'eye' icon appearing in my notifications.

I have turned off Smart stay and Auto adjust screen tone. I've set the screen mode as standard and Daydream is off. Yet, I still get the eye icon appearing several times a day for brief moments. I really want this icon gone permanently.

Welcome to Android Central! Did you also turn off Smart Pause?

Yes. All motions and gestures are off except smart alert. Air view is also off.
 
Hi there! I followed the instructions and cleared the cache successfully but the 'eye' icon is still appearing periodically. I ran an app called 'Hidden device admin detector' and it didn't find any malicious apps. My phone is only a couple of months old and only has very generic apps installed. VLC media player is the probably the most abstract video app I have. Is there any other advice you can give other than a system reinstall? - Thanks :)
 
I believe ive possibly found the solution/problem. The 'Smart Rotation' feature that allows the phone to rotate depending on the orientation of your face was on. Once I turned it off the eye has gone. Well, so far so good anyway. I dont ever recall turning it on but there ya go. Thanks for your help.

What that means is an app has taken control of your camera and is using it

Yea. Same for me. Inside screen rotation there is as a check off to activate Smart Screen rotation. Uncheck that and the eye disappears. You still get screen rotation when you turn the cell to landscape mode.
