Has anyone filed a claim with Nexus Protect?

Just wondering how this whole process will work if I ever need to use it. Do they send you a phone first or do you have to send them yours first? Do you send in the phone or everything including the box and all accessories? Similarly, do they send you just a phone or a new boxed phone, or is it a refurb?
