Help! My SD card blank?

Have had the SD card in my Note 3 since I bought it in Feb 2014. Yesterday it started having a SD card in the notification bar and when I look at it it says that the SD card is blank and asks me to format it. What is going on I've had the same SD card since I bought the phone and its always worked great. Please any help would be appreciated.

Do you have an adaptor to plug into a PC or Mac to check? Might be way to start troubleshooting.

Definitely want to get it to a PC and save it if it's still possible.
