Hosed my SD card?

So I was restoring my phone to try to fix the issue of puzzle lock screen not showing the message puzzle piece. Thought I'd completely restore to stock and re-configure it bit by bit to see where it was lost. Wiped the data and cache, and while I was restoring System with Odin, I formatted my SD card on my PC. With everything restored, I put the SD card back in and it said it had an unrecognizable format. It gave me the option to reformat it on the phone, which I did, but after rebooting, the card now fails to show up in the phone at all - it says no card is inserted. Tried it in my desktop card reader, and that fails to even notice the card is inserted too.

I assume it's shot, if I can't get anything to even see it, I can't even attempt to format it. Any ideas, or has it given up the ghost?

can the computer see it in the disk management application?

Nope, neither Disk Management in Windows nor Disk Utility in Linux can see it... it's shot, headed to Best Buy in the AM to get it replaced. Hopefully they'll just replace the SD card, because the phone is working perfectly.
