How do applications and games get backed up?

In the accounts and backup screen in the "backup my data" tab it talks about backing up passwords, call history and app data such as settings and files stored by the apps. Are all the apps by default backed up as well or is there some other option I have to enable.

It depends on the app. Google apps are tied to your account and some of the information is stored on their servers. Apps like Keep, and Calendar are good examples.

Google Play Games can backup some game data, but it depends if the developer has enabled that.
