You buy a phone and go to a carrier and set up an account, or you go to a carrier, set up an account and buy the phone from them.

If you do it the first way, make sure the phone you buy will work with that carrier.

(Choose the carrier by whether it covers the places you need covered - that's always the first thing you do. Then choose among all the carriers that have good coverage where you need it, if there's more than one, by plans and prices.)
