How do I get rid of the window at top of this screenshot?

The actual screen on my phone is full of apps. Not sure this one only shows the "MENU" button at the top of the screen. How do I get rid of the "MENU" button?

It pops up very infrequently. This sounds like a stupid question but how do you turn it off?

Go to settings>accesbilty menu tap>turn off accesbilty menu

It might also be from some 3rd party app. Does it go away in Safe Mode?
