How do I make my Note 5 lock screen work properly again?

Recently I discovered that my lock screen can be unlocked simply by swiping the screen. This is surprising because I have it set to be unlocked only with a PIN or my fingerprints. I don't know how long it has been like this, but I know it used to work properly in the past. I tried changing the security settings to swipe, then back again to PIN, but this didn't work. Anyone familiar with this issue and how to fix it?

Thank you. No, I do not have a trusted device or location. I checked just to make sure, but I've never set one up because I prefer to have my phone secure at all times.

Well, I figured out what happened. It turns out I had a trusted device (my watch), but I don't recall setting this up and I didn't notice it earlier when I checked my settings. I must have forgotten that I did this. I discovered it this morning, while my watch was charging and I had my phone in another room; the lock screen required my PIN or fingerprint until I brought it closer to my watch.
