How do I remove the Warning sign CAMERA FAILED on my Galaxy Mega 2 phone

I have tried to follow some suggestions on the internet but when I am told to find the camera icon in my application manager I cannot find it.

Same as when I connect phone to Computer, I don't know where to find the files suggested.
