How do I replace an App icon that disappeared?

The icon for an installed app no longer shows. How do I replace it?

do you mean it shows just the default android icon? in that case a reboot of your device or a re-install might help

Also, if by any chance it's an LG G3, it might have been dealt with by Smart Clean as an idle app, in which case, check for it via the Update Centre app.
