How do I stop my phone from continually ringing?

Phone keeps ringing

the phone will start to ring (no incoming call happening) ring for 15 seconds then stop, starts up again in 10 min. How do I stop this and how do I get the "chirp" back when I have an incoming text.

I have the exact same problem with my Samsung galaxy s4 active! it's been that way for about 2 months now ;/
