How Does Getting An "Advantage" Replacement Work?

So on my 15th day of ownership, my M8 slid out of my pocket, face down, and slid about five feet on a wooden floor and I guess there was the perfect amount of grit because there's 2 major gashes and a ton of hairlines. How does one go about getting their screen replacement?
