How to change SDHC cards?

I have a new, larger, micro-SDHC card I want to put in my NexusOne, replacing a smaller card.

Are there any How-Tos, Do's& Don'ts, etc for how to copy data, apps, etc, from the old card to the new one?

There's really nothing to it, assuming that you don't need to perform any cleanup. Copy everything from the old card to your PC. Copy all to the new card.
