I agree with mustang7757, it's likely related to some app you installed. "Phone cleaner" apps are most likely, but other apps can sneak this kind of junk in as well (like file explorers, battery savers, gallery apps, etc.). You may have to uninstall apps one by one until the problem goes away.

The app in that screenshot isn't disabled, and can't be disabled.

Regardless the process is go to settings, apps and find the app you disabled, tap it and tap the enable button.

That's your launcher , and cant be disabled.

If you cleared data and cache it will reset your home screen , its already enabled as u see you can't disable its greyed out

It looks enabled. Maybe you just cleared data for the app which means deleting your home screen setup.

Unless you are seeing a command line interface on your screen, you didn't disable the UI app. Now, a lot of the stock launchers do crazy things, like delete everything, and rarely is there anything you can do to recover it. Best you can do is set it up again, or use a different 3rd party launcher like Nova, set it up, and then Nova can backup the setup.

I installed Nova and still have the same problem .

FYI, when you see a greyed-out Disable button, that means the app can't be disabled -- it's always enabled.
