How will I connect my Samsung gear vr to my Samsung note 5 through occlusion app the second time?

Before I can still connect the two device but after I send my phone to be fixed i can't anymore.I try to restart my phone but I still can't connect.I download already an occlusion application but still can't connect.It doesn't have any samsung gear vr welcome screen.I don't know how to connect it.is don't have a gear vr application also I don't know how to install.
