Is it malware on Nexus4???

Hi guys,
Use Nexus 4 (Android 5.1.1, LMY48T) for about 2 years.
No problems during this period.
But about 2 months ago due to unknown reason (maybe virus or any other promblem) Nexus
started to behave abnormal.
Try to describe ordinary situation - boot up nexus; in 2-5 minutes nexus begins to independently start phone/message/viber/any other app -> calls/writes sms/ -
- simultaneously it swithes on/off Torch/Location/Aeroplane Mode and so on. It repeats many many times.
I can't deal with phone. And reboot it. After reboot situation repeats again.
Antiviruses didn't find anything suspicious. Frankly speaking all of them were free.
Week ago i carried out full factory data reset and refused to recovery application installed.
It didn't help.
Please help me in solving of my problem!

Carried out factory reset both from setting menu and recovery menu.
When i push power button, only Power Off is on the screen (no chance to go in safe mode).
Rooted nexus, flashed with custom ROM - problem remained. =(((
Can u tell me where to dig ?
 
When you see Power Off on the screen, are you touching and holding that Power Off message until you see the Safe Mode prompt?

Thank you for exact explanation how to enter safe mode.
Now nexus is in safe mode. Unfortunately the same problem with nexus persists in this mode.
Additional info: using Multirom TWRP 2015-06-30 (v.2.8.7) i wiped all data (besides internal storage) and.... bug exists even in this mode (TWRP).
Only questions - how? and what is it?
What should i do next to eliminate my phone bug?
Is it possible to wipe internal storage and then install/flash any custom rom?

Oh, I didn't know it was rooted and running a custom ROM. Did you install the ROM? I'd suspect a problem with it. Can you flash a different ROM, or the stock ROM, and see if the problem persists?
 
I flashed both stock and custom ROMs. Nothing changed.
Problem persisted.

You mean wipe as in completely overwrite, as opposed to what happens with a factory data reset? I'm not sure about that.

If this were malware installed to the root, I would have thought that reinstalling the firmware would get rid of it. I wonder if it's more of a hardware problem than a malware/software problem.
