Is my phone hacked or bugged?

My girlfriend has two numbers, in recent time when she used to call me from her primary number she started getting caller tune, and when calling from secondary number there was no caller tune. Now suddenly she here's the caller tune from any of the two numbers she calls from. It's like someone has hacked the phone and has added both the number to the list which is getting recorded.

I never have kept any caller tune to my phone. Is it possible that someone has hacked my phone and able to listen my conversation? Please give me solution if so. Need help.
