If you have set up a Samsung account, it's saved in the cloud. Open on the Samsung Galaxy S5 the app S Health. Then it is open you can see at the top right a three-point symbol. Tap on it and it will open a small menu. Select the entry "Settings". Tap next on "Accounts". Basically S Health data is secured by the Samsung Account. So now select your Samsung account in the account list and hit restore.

Welcome to Android Central!

Just make sure you are signed in to your Samsung account and your sync is turned on in S Health.
