If I had to guess, I'd say it's probably some app that had been previously installed and was incompletely or incorrectly uninstalled, or else it might be something that was originally preinstalled on the phone but then lost support. That Android icon is often a null placeholder for something that either doesn't have an icon or somehow got borked and is no longer functional.
