just a Nexus 4, need some help with questions please!

1) when you get the nexus 4, it comes with a plastic front screen protector AND BACK protector pre-installed correct? or does it only come with the front protector?

2) so i had a problem when making a call and the screen did not wake up. it turns out to be a problem with my screen protector and the proximity sensor. what are some good affordable screen protectors with proximity sensor cut outs?
