Location keeps turning off.

I'm having a very annoying issue. For some reason, Location keeps turning itself off. I turn it on (High accuracy or power saving, doesn't matter) via either the menu button or by going into Settings -> Location and flipping the 'switch' to On, and invariably at some point when I unlock my phone, the pop-up "Location services are turned off" with the choices to turn them on or remind me later is waiting for me.

I'm not using any power savings settings (I know there's a bug with UPSM that doesn't auto turn the setting back on) or any Apps I'm aware of that would turn off location (especially given I have programs like Avast and Android Device Manager that want location on to find the phone!). I did use USPM in the past at night, but I decided I wanted to see how long the batter would last without it, and I haven't enabled it in almost two weeks.

Anyone have similar problems who's found a solution? I've tried clearing the cache partition and I *really* don't want to factory reset my phone, but its almost getting to that point I think.

There are a few threads about this but people describe it as a GPS issue.

I don't have a solution either. I googled for an answer. I also have the Note 4. My Note 3 was so much never did this. So frustrating. Please fix it. My other issue is that after a phone call I set my phone down and it redials the person I just hung up with.
