Lock Screen Changes Styles/Display

Hi all, new person to forum!

I noticed yesterday that my DNA has been acting peculiar, but I have not seen any other posts that are similar. This started to happen just before and ever since the 15.5MB update that occurred yesterday.
What happens is that when I turn the display on on my phone, it will change which standby/lock screen to show. The one with the circled lock I have never seen before. Here are some screen caps (my wallpaper changes, so disregard that part):

Normal Screen:

New Screen:

I've also noticed that it does not make me unlock the phone in any way - just slide the ring or circle, and I'm in. Just experimented with the settings for the lock, and changed it to "lock immediately" to test. It worked: showed my old screen, with my connect-the-dots option, then went to the new screen followed by going back to the connect-the-dots.Thoughts?
I'm just wondering, primarily, where this new lock screen is coming from - my home menus look normal, not in this new style.

Interesting, I'm not sure that I realized the DNA had the option for the AOSP unlock screen.

I assume you are running stock, haven't put a custom ROM on your device or installed a lock screen replacement?

Correct, I'm running nothing custom on my phone. Settings for locking/unlocking do not show anything new; phone still does not lock when I have it set for my normal 10 minute wait period. I'd put it back into the "immediately" setting, but I use my phone often, and would get tired of unlocking ...

Okay, that's new. I just changed the setting back to "immediate", and it is no longer working either. Turned phone display off, turned back on, and still showed new unlock screen and did not even ask me to connect the dots for access. Have not seen the old unlock screen since yesterday.

Android Version: 4.1.1
HTC Sense Version: 4+

Do you have a no lock app installed...maybe it is acting up or turned to no screen lock...never saw this otherwise...if all else fails factory reset.

I did not have a no-lock app installed, but I did find out what the culprit was!
There was an app or widget causing the weird stuff, which I did not recall ever installing. I did not see the icon for it on my homescreen until a few days after the issue started. The weird part was that I removed it from my homescreen, not an uninstall, but it's nowhere to be found on my phone and the issue is gone. Completely, as if it never existed. I don't even recall what the name of the thing was called.
So, issue resolved!

Here are some screen grabs, in case anyone recognizes it: 2013-10-08 18.27.35.jpg 2013-10-08 18.27.54.jpg
It had a weird sidebar thing that I discovered you could configure, as well as set a lock screen via the connect-the-dots, PIN number, etc. that was not from the main phone settings. That was why my lock was being disabled, because a lock was not set with this app/widget thing, which in turn overrode my settings.
