Missing app icon on Galaxy S6, how can I get it back?

Missing app icon on Galaxy S6

When I go to "Settings," "Applications" and then "Applications Manager - All," there is an "NFL" app installed but no icon for it and no way for me to launch it. There is no option to Uninstall it. I tried to disable it but got a message that said disabling pre-installed apps could potentially cause problems with other apps and that all data would be deleted. I even tried going to Google Play and re-installing the app, but it says that it is already installed. It does give me the option to open the app from there, but that's a long way to go just to launch this app. Any suggestions as to how to correct this?

So the app isn't even showing in the app drawer? Look through all the folders too.

Have you tried restarting the phone?

It does not show up in the Apps drawer and I have tried restarting the phone.
