MOTO G Restricted Access Changed Help Needed

I am always getting this Alert Notification after every 10 Minutes and by pressing this alert my Data signal disappears.

PLS help.
I have rebooted my phone, done the factory reset but still the same problem persists.

this might happen if the sim is not inserted properly...remove the sim card n put it back after two mins n it will be fine after tat
