Moto X Pure SD Card Problems

So about a week ago I noticed my Moto X Pure was doing something odd. I have a 128gb SD card installed as internal storage. All of a sudden apps have started installing on the internal memory instead of the card. Whenever an app gets updated it now moves to the internal storage. I have had the phone and card since it launched last September. I did a factory reset the other night and it is still doing it. I am running Marshmallow, stock, non rooted. Thanks in advance!

What card do you have, and where did you purchase it? Many cards sold on eBay are fakes.

It's a SanDisk Ultra that I got off Amazon. I have tried a Lexar Card also and the same thing keeps happening. The phone see's the cards but it starts installing things on the internal.
