I have a Note 4 with AT&T running Lollipop 5.0.1

Welcome to Android Central! After those first 15-20 seconds, does the autofocus start working normally? Or is it still sluggish? Is the camera lens completely clean? Any recent trauma that might have affected the autofocus mechanism?

This is a known defect in the note 4. I know people who have received replacement phones for this issue. If you are able, I would contact the carrier/retailer where you purchased the phone, or Samsung, explain the problem and request a replacement.

Yes, after the 15-20 seconds it starts working fine, until exit out of the camera and open it again. The lens is clean, and there hasn't been any trauma to the phone.
