Welcome to Android Central! Use a can of compressed air to blow out the headphone jack, and root around there with a toothpick to make sure it's clear of debris. If it's clear and the problem persists, then the jack is probably faulty, and needs to be replaced. You could try a factory reset first, but if the problem persists, then it's most likely a hardware problem.
