can you include a picture of the app you're trying to remove? If it's a system app, you won't be able to remove it/disable it from your phone (some Verizon apps might do that).

Could it be the s health notification? Open the notification panel, tap on the notification so it tells you what app it is. You can then change the settings or disable notifications for the app all together

Was it from a Facebook friend sending you a message from Facebook messenger?

If it is, just drag the circle towards the bottom of the screen just above the home button. There should then be a white circle with an X inside of it. Just drag the man's face into the circle to remove it from your screen.
