nexus 4 here, Lollipop update gives me a new Android icon ??

I believe that's the icon that shows up when an icon is left on your home screen for an app which is no longer installed. If I were you I'd just remove it.

Yup... what Zorak950 said... it is a "dead" shortcut basically, the app it pointed too is missing or changed significantly, just delete it and add back what you like there.
