It's a Zooper widget by Beard that you can find in the play store. You will need the paid Zooper to use it.

Here's what I have for now. I keep a handful of my most-used apps front and center, and prefer dark backgrounds to keep icons from blending in too much. This is just Nova Launcher with Lollipop icons and Beautiful Widgets 4x2 with the Roboto Clear clock and default weather.

Any suggestions on icon packs to complement this wallpaper?
