Accessibility menu toggle off when you open accessibility>interaction and dexterity turn off assistant menu

That looks a little like an Edge Panel. You might try going to your settings, search for edge panel & see if it's toggled on.

If nothing else, you'll be able to rule that out as a possibility.

I tried to create that exact layout, but I couldn't quite achieve it. It looks a bit like the task edge panel. The layout could be different from what it was on the Note 10/ 10+.

But I agree with you that it's probably a 3rd party app.

It should of went away in safe mode as it turn off 3rd party apps unless device admin app
