"OK Google" voice activation, deactivated?

SOLUTION

SYMPTOMS -
• Microphone button is empty (outline only)
• Hotword Detection is enabled and fails.
• Tapping the microphone button is required
• English(US) language setting selected

SOLUTION -
Settings > Accessibility
Scroll down to "Services"
Disable services that change/use/control sound and/or microphone.


FOR ME:
The Sound Profile (an app) service was the issue.
For me, The app functions properly without this service running.
 
One of the culprits is S Voice. If you have S voice activated it will screw up the "ok google". Turn it off and the "ok goggle" will come back to your home screen. You can find out if it's running or not by checking via settings/applications/running applications/S voice
 
OK GOOGLE does not work from the widget.
I was facing the same problem. Initialy the "ok google" was working from the widget and suddently the back microphone disappeared and I had to push the microphone to get into google to use the "ok google function". Seems that the problem was with my launcher (NOVA) it has an additional setting to let you use the "ok google" function on the widget.
Try NOVA SETTINGS > ok google > enable "ok google" hotword and its back! Working!
 
**FIX FOR OK GOOGLE WIDGET PROBLEM**

Clear Cache and Uninstall updates from the Google App. Reinstall Updates and you're good to go 👍
 
Try this it worked for me.

Settings>Sound>Volumes

Make sure that the volume for "Music, video, games, and other media" is at maximum

This solved my problem. Hope it helps you too. I had no idea these things were connected.
 
Settings → Language & Input → Voice Input → Enhanced Google services [CogWheel] → “OK Google” detection → From any screen [ON]

Broke for me when updated to 5.1.1 from 5.0.x. This fixed it.
