OS has been modified...??

I rebooted my phone this morning and noticed an icon of an open lock & the word "Custom" on my boot screen... I have not rooted a phone in over 2 years and have been through several devices since then! It wont let me check for updates because of this. Is this happening to anyone else? What could've caused this?? I do not sideload apps and unknown sources is always off.

I am on the S7 Edge from T-Mobile

Are you using Package Disabler Pro to disable apps?

I do have it installed. Haven't used it at all. I originally got it to use with my VR Headset but never blocked any services... Will un-installing fix the problem?

It has been on my phone for a month or so and this is the 1st time I'm seeing trouble..
