Otter Box Defender / Commuter feedback?

This is my first time using an otterbox defender case. My phone is an LG V10, but I just wanted to have a more open discussion for any otterbox users. What have been your experiences with these cases? Has anyone broken a phone while it was cased in an otterbox ? For those who have, what kind of drop, bend or impact caused the damage?

Do you use the OEM clear plastic screen protector?... or some other way to protect the glass?

My phone is a V10, with an otterbox defender. But I was just wanting to get a more general consensus from all users of the defender/commuter cases.

I've never used a Defender, but I've used a Commuter on my 2013 Moto X. I did not use the plastic screen protector. I used a tempered glass screen protector that I bought from Amazon. I never had any damage to my device, but I never dropped it either.
