What the hell is that?
And I have another problem when I open camera sometimes it tell me that I need to put SDcard to take a photo, what is that too? How to fix it?

Welcome to Android Central! Sorry for the delay. Is your Nexus 5 rooted or unrooted? Have you discussed those Restricted Access warnings with your wireless carrier? That's the first place to start, considering the blocked services have to do with the wireless service. Maybe the SIM card is bad.

Not sure why the Camera would be telling you to insert an SD card (which is why I wonder if you're rooted and running a custom ROM). Have you tried booting into Recovery and wiping the system cache partition? Did you delete a folder called "storage"?
