Question About Camera Flash

When the camera flash is set to "on", I thought this meant the flash would fire each and every time regardless of ambient light, i.e., so it could be used for fill-flash situations even in bright light like a DSLR. Mine did not fire each and every time, but maybe it's because I had the default HDR setting to "auto".What am I doing wrong? In a bright lit room, with HDR off and flash to "on" it seems like it will fire sometimes, but not every time sort of like an auto setting.
