When updating the same ROM, is a complete wipe necessary? Or is wiping cache\dalvik sufficient?

Sometimes just wiping the cache would be enough but it all depends on how many changes and what those changes are. If its a real minor change you won't have any issues but most update aren't just a few changes. It's always best to wipe data unless the developer tells you different.

Doing a full wipe is always the best option, but at the very least you should wipe cache/dalvik, and probably /system as well.

Titanium Backup or MyBackup Pro

You would need to use a different launcher to backup home screens and folders, try Nova launcher which is almost the same as stock with a few extra features like being able to backup then restore. Just Google Search Nova Launcher and you will find a download somewhere.
