remote health monitor notification

Looking for an app that works with android wearable smartwatch that monitors vitals e.g. heart rate, that can notify someone via text etc. of problems. Many apps e.g. Google Fit seem to monitor health/fitness stats but I can't find one that can notify someone of problems/issues
