Remove Verizon Caller Name ID... HELP!

For the life of me, I cant stop the Verizon Caller Name ID app from popping up every single time I get a call.

It always tells me the free trial has expired & asks to purchase for 2.99 a month. I click "No Thanks". Then on the next screen, it tells me, if I continue, I will no longer be able to identify unknown callers. I click "Continue"... then it gives "Trial Ending" and makes the phone look like its processing. Then it takes me to "Unable to complete request. Please try again later."

So frustrating... stupid bloatware.

Any ideas?

You can disable it in the settings - applications menu. App is called "caller name id"
